Instructions for Filling in the Form on your computer (or tablet/phone)
This form is designed so that you can type the information directly into the form on your device, there is no need to print it. If you have problems filling it in on your device as an alternative you may print it and use a pen.
- Request the Membership Application Form by clicking here.
- Open the form and fill in all the usual details.
- Please remember to tick boxes, for Volunteering, Gift Aid if appropriate and one of the payment methods.
- Please also remember to sign (typing your name in the signature box is fine) and date the form.
- Important Please save it to your computer using your Surname and Membership Number(s) as the file name. If you are a new member just use your Surname.
- Email the completed and saved form back to me at membership@thetfordu3a.org
- Make your payment - Payment is by BACS (preferred), cheque or Credit/Debit card (at monthly meetings or other prearranged events). No cash please.
